04/13/2026 – 07/11/2026 (Start 4/13 or 4/27) Schedule:
Monday–Friday, 8:00 AM – 4:40 PM Hours: 40 hrs/week guaranteed
On-Call/Weekends: None Requirements - Active RN License - BSN
required - Certified Care Manager required - Minimum 3 years
inpatient clinical experience - At least 1 year of Care Management
experience Job Responsibilities - Manage daily inpatient caseload -
Attend interdisciplinary rounds - Complete initial assessments -
Develop and coordinate plan of care with patient, family, and
healthcare team - Coordinate post-discharge needs (home health,
inpatient rehab, DME, nutrition, etc.) - Communicate with medical
teams and post-acute referral sources - Document and place orders
