Travel Position Physical Therapist Assistant (PTA) – Home Health
Work for North Country Home Health & Hospice Agency Patients can
range from: Pediatrics ? Geriatrics Required Experience Candidate
must have: ? 1–2 years current PTA Home Health experience ? Home
Health or Hospice experience ? Experience with all age groups (Peds
? Geriatrics) ? Previous travel experience – Highly preferred
Documents Required for Submission Submission file must include: -
Work history - PTA skills checklist (within 1 year) - 1 supervisory
signed reference - License verification - Permanent address
Certifications Required - BLS certification from the American Heart
Association License Requirement - Active Physical Therapist
Assistant license from the New Hampshire Office of Allied Health
Professions All licenses and certifications must be active at
