  • High-School Diploma or Equivalent
  • Minimum 1-2-Years Healthcare Experience
  • Minimum 6-Months Home-Healthcare Experience (Preferred)
  • English- and Spanish-Speaking
  • Knowledge of Basic Healthcare Terminolgy (Preferred)
  • Application-Based Technology Skills and Web-Based Software Competence
  • Effective Communication Skills via Multiple Channels (Phone, Email, In-Person, etc.)
  • Organized, Concise and Detail-Oriented with Goal-Oriented and Team-Based Approaches to Multi-Tasked Solutions
  • Flexible, Adaptable and Creative-ThinkerResponsibilities:
    • Act as main contact for patient access
    • Coordinates core elements of patient communications
    • Creates and maintains patient schedules in accordance with physician orders and clinical needs within the scope of Agency policy
    • Monitors, maintains and adjusts clinician schedules to accommodate variations in staffing capacities; such fluctuations may include: Call-outs, vacations/PTO, over-productivity, etc.
    • Verifies schedule activity is created, adjusted and maintained in alignment with local, state and federal regulatory guidance, as set forth by: Local Department(s) of Health practice acts; CMS Conditions of Participation rulings; And the standards of applicable accrediting bodies
    • Facilitates the coordination of transitional care dynamic(s) within the scope of their designated role to schedule home-health admission (SOC) visits and initial comprehensive assessments within designated time-frames
    • Maintains reporting data as required on a daily, weekly, monthly, and otherwise directed basis
    • Actively participates in collaborative discussions and meetings
    • Performs daily schedule reconciliation
    • Confirms visit(s) with patients/clinicians via telephone; Documents applicability within the patient's medical record
    • Coordinates and assists patients, families/caregivers, other members of the care team (physicians, labs, pharmacies, etc.) via telephone; Documents applicably in the patient's medical record
